Provider Enrollment
Our goal is to have a large, open network of well-qualified
providers. We desire our providers to embrace our model of patient centered care.
To be a provider in our programs, a healthcare professional must have completed a required
certification program. A Network Agreement
contract must be completed with the ND PSC to become an enrolled provider. Upon execution of the above, providers must complete
approximately one hour of additional training on the clinical software system within 30 days after assignment
of their first patient. There will be
additional continuing education requirements to maintain certification that will be established by the
Advisory Board.
Providers are expected to provide quality disease state and medication therapy management
services to enrolled patients. This includes
maintaining an acceptable environment for service provision such as a private consultation room, high speed
internet access, blood pressure equipment, and provided patient education materials. Providers need to have dedicated time in their schedule to
meet individually with patients. Support staff,
such as pharmacy technicians and interns, can be utilized for much of the documentation and data
collection.
Program compliance is essential, both from the patient and provider standpoint. The patient and provider are responsible for making and
keeping scheduled appointments. The patient
appointments need to follow the visit outlines provided in this manual. Educational materials used during visits and provided to
patients should be supplied by the clinical coordinator or accessed from the program website. Providers need to notify the clinical coordinator when
patients do not keep scheduled appointments.
Documentation is essential to program success. Providers must collect the pertinent data detailed in the
provider manual and document it in the clinical software system within one business day of the
visit. Reimbursement will not be provided
unless all documentation is provided. In
addition to entering visit data and results in the software system, providers also must provide written
documentation generated by the software system to the patient, including a visit summary, goals established,
and a personal medication record. This
information should also be shared with the patient’s primary care physician and any health professionals to
whom referrals were made.
Resources will be available to providers. The program website is a valuable source of information as
well as a means to allow discussion between providers. The website also contains all program forms and patient
education materials available for download and printing. The clinical coordinator encourages questions and feedback
on the program as continuous evaluation and improvement is desired. Educational opportunities will be presented during various
professional association meetings.
