  Our services are available to anyone. Providers can provide you with claim forms to submit for reimbursement from your insurance of FLEX benefits.

• NDPERS Program
  
  Our diabetes management program is open to any NDPERS member that is diabetic and active in the health plan.
• CHAND Program
  
  Our diabetes management program is open to qualifying CHAND subscribers.
