Our Services


Our programs focus on medication therapy and disease state management. We provide services such as health risk assessments and biometric screening to identify those who would benefit most from our more comprehensive programs.  Our programs are based on successful, established guidelines for patient care. We aim to work collaboratively with physicians and other specialists to improve our patient’s health and improve their quality of life.

Diabetes Management Program

The Diabetes Management program focuses on helping patients better understand and control this harmful disease. There is a series of up to six visits with a designated provider, which the patient selects, over 12 months. The visits take from 30-60 minutes to complete. Each visit involves an overview of medications along with an educational theme for each visit. The provider will review the patient’s health history, diabetes goals, recent lab results, and all of their current medications. The provider will then educate the patient on their health conditions, make recommendations to improve their medication therapy, and refer them to other health care providers such as diet or eye specialists if needed. All information will be shared with the patient’s doctor to make sure that everything is done to meet their medication and health goals. The patient’s health will be improved by utilizing the specialties of a variety of health care professionals.

There are multiple benefits to this program. Most importantly, patient’s overall health will be improved.   Secondly, health plans can reduce expenses due to having healthier member, less sick days, and less long-term disease complications. We can also customize this program to meet your specific needs.

Health Risk Assessments

Our website provides a portal for health risk assessments (HRA), general health assessment, diabetes assessment, cardiac assessment, fitness assessment, and calculators for body mass index, basal metabolic rate, ovulation estimator, ideal weight, smoking costs, and others.  Members have their own personal login for private access and viewing of results.  Each assessment exam includes instant online results for the member.  Useful website links are provided in the report for the member to gain additional information on conditions the assessment identified as at risk.

Biometric Screening

Our provider network has pharmacists and pharmacies with the ability to offer CLIA-waived testing of cholesterol, blood glucose, hemoglobin A1C, body mass, bone density, and others.  Onsite health screenings are a valuable tool to identify those at risk for chronic health conditions such as diabetes, osteoporosis, and heart disease.  Tests require no more than a prick of the finger and results are received in a matter of minutes.

Medication Therapy Management (MTM)

Our comprehensive MTM program is designed to optimize the treatment regimen of a patient.  It involves a comprehensive medication therapy review, education on medications and wellness issues specific to the health of the patient, intervention and communication with other healthcare professions, development of an action plan with patient established goals, a personal medication record, and follow-up.  Our certified pharmacy providers utilize evidence-based guidelines to optimize the health of the patient.  Our MTM/DSM program participants will be seen a approximately four times over the course of a year. 

Immunizations

In North Dakota, pharmacists that have completed special requirements are able to provide immunizations to those age 18 and over.  Some vaccines of potential interest to adults are influenza, hepatitis, and tetanus.
