Eligible subscribers are automatically enrolled in the program.  If you would like to inquire on your status, please contact the clinical coordinator at Frontier Pharmacy.  If you are enrolled and would like to opt-out or change providers, please click above.

Self-pay patients

Anyone can use our services, even if your benefit plan does not participate with our group.  You will be responsible for the charges for your appointments with our providers.  Your provider can provide you with billing statements and forms that you can then submit to your benefit provider to see if they will provide any reimbursement.  You can also submit the forms to utilize your FLEX spending account.
