CHAND Subscribers


The Comprehensive Health Association of North Dakota (CHAND) is pleased to announce a Diabetes Management program. This service will be provided by pharmacists and other health professionals throughout the state. The North Dakota Pharmacy Service Corporation will be administering this program and they have contracted with Frontier Pharmacy Services for the clinical coordination of the program.

The Diabetes Management program will focus on helping enrolled subscribers better understand and control this disease. There will be a series of six visits with a designated provider, which the subscriber can select, over the next 12 months. The visits will take from 30-60 minutes to complete. The provider will review the subscriber’s health history, diabetes goals, recent lab results, and all of the subscriber’s current medications. The provider will then educate the subscriber on their health conditions, make recommendations to improve their medication therapy, and refer them to other health care providers such as diet or eye specialists if needed. All information will be shared with their doctor to make sure that everything is done to meet their medication and health goals. Their health will be improved by utilizing the specialties of a variety of health care professionals.

Subscribers will benefit from this program in multiple ways. Most importantly, the goal is to improve their overall health.   Secondly, subscribers participating and complying with the requirements of the program will receive a $25 benefit, paid quarterly, for each of the six visits with their designated provider. The entire cost of the program is paid by grant funding through the Centers for Medicare and Medicaid Services. Subscribers will not be asked for any additional payment from the provider beyond your cost share on prescriptions. Enrollment in the program is limited to 180 qualified participants.
